State DL 920 Form DL 920 Verification Of
Installation Ignition Interlock The DL 920 form is a required document provided by the Department of
Motor Vehicles. This form is given to all Smart Start clients by their authorized
Smart Start installer once the ignition interlock device
has been installed on their car. The DL 920 is the Verification of Installation
– Ignition Interlock. It is what a Smart Start client must bring to the DMV
to prove they have indeed had their Ignition Interlock installed. The DMV
will not recognize that the client has had their Smart
Start Ignition Interlock installed until they receive the DL 920. The DL 920
form includes driver information, manufacturer/facility information, ignition
interlock device information and vehicle information. The DL 920 is only available
from an authorized Smart Start installation site and is not valid without the
Smart Start manufacturer's stamp – also only available at an authorized Smart
Start installation dealer. DL 921 Notification
Of Non-Compliance Ignition Interlock The DL 921 form is a document
provided by the Department of Motor Vehicles. This form is sent to the DMV by
Smart Start Inc. when any Smart Start client is not complying with the legal
guidelines of the ignition interlock program. A DL 921 is required of Smart
Start whenever an ignition interlock has been bypassed, tampered with or removed
prior to the DMV ordered date. A DL 921 is also required from Smart Start when
a Smart Start client's car has been locked out by their ignition interlock within
any three months. Depending on a Smart Start client's personal
standing with the DMV, a DL 921 could result in suspension or even termination
of his or her's drivers license. The DL 921 form includes driver information,
manufacturer/facility information, ignition interlock device information and vehicle
information. This form is generated by Smart Start Inc. and not from an
unauthorized Smart Start Installation Dealer. DL 922 Ignition
Interlock Notice Of Removal The DL 922 form is a document provided
by the Department of Motor Vehicles. This form is given to all Smart Start clients
by their authorized Smart Start installer once the device has been removed from
their car. The DL 922 is the Notice of Removal of Ignition Interlock. It is what
a Smart Start client must bring to the DMV to prove they have indeed had their
Ignition Interlock removed. The DMV will not recognize that the client
has had their ignition interlock device removed until they receive the DL 922.
The DL 922 form includes driver information, manufacturer/facility information,
the device and vehicle information. The DL 922 is only available from an authorized
Smart Start installation site and is not valid without the manufacturer's stamp
– also only available at an authorized site. ID-110 Ignition
Interlock Installation Verification The ID-110 form is a required
document provided by the courts. This form is given to all Smart Start clients
by their authorized installer once the Smart Start device has been installed on
their car, if their ignition interlock has been court
ordered. The ID-110 is the Ignition Interlock Installation Verification form
that a Smart Start client must provide to the courts to prove they have indeed
had their ignition interlock installed. The courts will not recognize that
the client has had their Smart Start Ignition Interlock installed until they receive
the ID-110. The ID-110 form includes driver information, manufacturer/facility
information, ignition interlock device information, court/case information and
